1. Completed and signed ISSA form (PDF or Word) - fill out ONLY your personal information as I will do the rest
2. Two checks ($18 to ISSA for player's pass and $75 to The Soccer Club for club fees)
Subs: ($18 to ISSA for players' pass andl wait until the end of the season to pay club dues based on games played)
